MEMBERSHIP AGREEMENT

This membership agreement is entered in between Donofrio Fitness located at 370 Franklin Tpke. Mahwah NJ and "you" the member. Donofrio Fitness is a boutique fitness gym offering group fitness classes and Personal Training. Please review the agreement thoroughly as it is a legal contract between Donofrio Fitness and You, the member. By signing and attending class,events, and other programs you hereby acknowledge on behalf of yourself that you except all risks associated with the class and this facility.

COVID- 19 UPDATE

Health and safety remain the priority at Donofrio Fitness, we will also adhere to new guidelines in accordance with the latest NJ Governor's ordinances. Donofrio Fitness has always been dedicated to our community and supporting the health and fitness goals of our members. We have put in place preventative measures to reduce the spread of Covid-19 and its variants and as always will continue to disinfect all high touch surfaces and mop gym floors between classes.

DONOFRIO FITNESS DOES EXPECT IF ANY MEMBER IS SICK, PLEASE STAY HOME.

FREEZE POLICY:

Members with monthly unlimited memberships may put their membership on hold in one month increments for up to 2 calendar months for a fee of $30.00 per month. Members will not be billed during the freeze months. Billing will resume  automatically upon the end of the freeze month.

CANCELLATIONS & BOOKING POLICIES

The Donofrio Fitness App is solely used for booking and cancelling classes. Members may also call the gym if the class begins within 1hr. of their booking.(201-252-2259) Signing up for class is MANDATORY, you cannot just show up to a class. Cancelling within 1hr of a class, members will be subject to a $8.00 fee. Please adhere ro the above requests, this ensures the Trainers have adequate time to plan a great workout.  

PURCHASES AND PAYMENTS   

Monthly memberships are paid in advance on the calendar date which the membership became activated for the following month. Month to month no contract is paid in full at the time of sign up along with all other class packages. contracts will automatically renew unless notified by the member. It is the members responsibility to give written notice 30 days prior to to the cancellation of their contract. Members who have purchased a 6 month unlimited membership are obligated to fulfill the entire contract. If early termination is required the member is still obligated to pay the remainder of that contract in full.

REFUNDS

We are no longer able to make any exceptions for accidental purchases. If members accidentally book and pay for a drop in, we will offer you a credit on your account or discount against your next monthly payment. 

PHOTOGRAPHY

I am aware the classes the member or my child attend may be streamed live to participants attending online, and sonme classes may be recorded for later viewing by members or clients. In addition, some pictures may be taken. I am aware that the member or my child may appear in these videos/images. If the member or my child do not wish to appear in the video or in pictures, I will let the instructor know before the class. Failure to inform the instructor means that the member, or my child can be in the video or livestream or images for use by the studio
